Simulation Fidelity
Simulation fidelity is the level of realism of a simulation activity, meaning the degree to which it reproduces the reactions, interactions, and responses of the real event or workplace. The Healthcare Simulation Dictionary describes fidelity as multi-dimensional, covering physical, conceptual, psychological, environmental, and emotional accuracy, and notes that it is not tied to any one modality. Standards deliberately decline to prescribe a level, because the right level is whichever one achieves the expected learning outcome.
Splitting fidelity into dimensions is what makes it usable. Physical fidelity covers how the equipment and environment look and feel, conceptual fidelity covers whether the case hangs together clinically so that the vitals match the diagnosis, and psychological or emotional fidelity covers whether the learner behaves as they would with a real patient. A scenario can score high on one dimension and fail on another, and the failure is usually conceptual: a case whose numbers do not fit its story teaches learners to game the simulator.
The Healthcare Simulation Dictionary records that participants and educators prefer higher fidelity and judge it superior, while the evidence does not support that contention, with all levels of fidelity found beneficial when used appropriately. INACSL standards take the same position, declining to dictate a level and asking instead that realism be set at whatever promotes the expected learning outcomes.
For high-stakes conversations, emotional fidelity carries the load. A learner practicing a serious illness conversation needs a patient who is genuinely hard to read, who pushes back, cries, or changes their mind, because the skill being built is reading and responding rather than reciting a protocol. Programs that raise physical fidelity while leaving the emotional content flat get learners who perform the checklist and freeze at the first unscripted reaction.
The operational version of all this is a design question asked before any equipment is booked: which dimension of realism does this objective actually require. Once a program answers that, the modality choice tends to be obvious and the budget argument gets much shorter.
What this looks like in a program
- Case authors review new scenarios for conceptual fidelity, checking that vitals, labs, and imaging tell one consistent story, since that is the dimension most often broken in a rushed build.
- A program that needs emotional fidelity for communication assessment invests in trained SPs, AI patients, or both, and stops treating the manikin room as the default venue.
